Repeat Prescriptions

In our ongoing effort to make your pet's health care as convenient and easy as possible, you can request a refill for your pet's prescription by submitting the following form. Please be sure to fill in all the requested information. The prescription refill must be approved by a doctor and we must have seen your pet within the last 12 months.

We will notify you when your request has been received and will contact you by phone if clarification is needed or if we have any other questions. Generally anticipate a half-day delay in picking up your prescription (ie. if you send us your request in the early morning, it should be ready by the early afternoon. If you send us your request late in the day, it will be ready the following morning). If you require your prescription more urgently, please call the clinic 306-545-7211.
